With intuitive dashboards, automation features, and actionable insights, the app is designed to help businesses grow revenue and strengthen customer engagement.
2. What Data We Access / Permissions
When a merchant installs our Shopify app, we require permission to:
- View personal data — Customers: name, email address, phone number, physical address
- View and edit store data — Customers: read and modify customer records
- Storefront tag visibility notice: tags you have added to customer accounts may be visible to customers shopping via our app
3. Purposes of Data Use
We use the data we access strictly for:
- Integrating app functionality with Shopify (creating/updating customer records)
- Operating features you select (e.g. form submissions creating customers)
- Supporting, maintaining, and improving the app
- Responding to user requests, support, or notices
- Ensuring security, preventing fraud, and enforcing terms
4. Data Storage & Retention
- We retain accessed data only as long as necessary to support your app configuration and features.
- Deleting or uninstalling the app will initiate removal of stored data (unless legally required to retain some records).
- Backups or logs may persist for a limited period, anonymized or aggregated where possible.

9. Refund Policy
If you purchase a subscription and are not satisfied, you may request a refund within 14 days of your initial purchase. To qualify:
1.You must not have engaged in spamming or any activities that violate our terms. Any evidence of spamming will disqualify you from receiving a refund.
2. For lifetime subscriptions, you must contact us within 14 days of your purchase to request a refund.
3. After the 14-day period, no refunds will be issued.
Refunds will be processed to the original payment method within 7 business days of approval.
4. We reserve the right to deny refunds if the request does not meet the conditions outlined above or if it is deemed abusive.

1. Introduction
FormCRM (“we”, “our”, or “the App”) is a Shopify app that allows merchants to create, manage, and embed custom forms across their storefront, including contact forms, lead capture forms, registration forms, surveys, and other customer data collection workflows.
FormCRM helps merchants collect and manage customer information through customizable, AI-assisted forms, automate form submissions, and integrate collected data with Shopify and supported third-party services (such as email/CRM tools).
This Privacy Policy explains how we access, use, store, and protect data when merchants install and use the App.
2. What Data We Access / Permissions
When a merchant installs FormCRM, we may request permission to access the following data, depending on enabled features and configuration.
Shopify account data access (as displayed on the Shopify App Store listing):
- Store owner’s data: name, email address, phone number, physical address.
- Customer data: name, email address, phone number, and physical address.
- Customer records: view and edit customer data (including creating/updating customer entries if the merchant enables customer creation).
Data collected through forms:
- Form submission data: information voluntarily submitted by customers through forms (for example, contact details, answers to survey questions, messages, and any other fields the merchant adds to a form).
- Files (if enabled by the merchant): if a merchant adds a file upload field, the App collects the file and related metadata submitted by the customer.
Storefront tags and metadata:
- Tags added to customer accounts may be visible to customers when interacting with forms or customer account experiences connected to the App.
We do not access or store payment or credit card information, government-issued identification, sensitive health data, or Shopify account credentials.
3. Purposes of Data Use
We use the accessed data strictly to:
- Enable form creation, embedding, submission handling, and storage.
- Create or update Shopify customer records when the merchant enables customer creation or syncing features.
- Deliver selected integrations that the merchant connects (for example, exporting submissions to Google Sheets/Drive or sending leads to email/CRM tools).
- Provide customer support and respond to inquiries.
- Maintain, improve, and secure the App, including implementing abuse prevention measures (e.g., spam and misuse controls).
We do not sell customer data or use collected data for external advertising, profiling, or marketing unrelated services without explicit consent.
4. Data Storage & Retention
Data is retained only for as long as necessary to provide FormCRM’s functionality and the features enabled by the merchant.
Merchants may delete form submissions within the App at any time.
Uninstalling the App will trigger deletion of stored App data, unless retention is required by law or needed for legitimate security purposes (for example, investigating abuse).
System logs or backups may be retained temporarily for security and operational purposes and are anonymized or aggregated where reasonably possible.
5. Data Sharing & Disclosure
We do not sell or rent personal data.
Data may be shared only in the following situations:
- With Shopify: to support App functionality and Shopify API operations.
- With service providers, such as hosting, infrastructure, analytics, email delivery, spam prevention, and customer support tooling, under confidentiality and data protection obligations.
- Legal requirements: when required to comply with applicable laws, regulations, or lawful requests.
- Business transfers: in the event of a merger, acquisition, or asset sale, subject to confidentiality and notice where required.

We are not responsible for the privacy practices of those third parties, and merchants should review their privacy policies before enabling integrations.
9. Refund Policy
If you purchase a FormCRM subscription and are not satisfied, you may request a refund within 14 days of your initial purchase, subject to the following conditions:
- You must not have engaged in spamming, abuse, or violations of our terms.
- Lifetime subscription refund requests must be submitted within 14 days of purchase.
- No refunds will be issued after the 14-day period.
- Approved refunds are processed to the original payment method within 7 business days.
- We reserve the right to deny refund requests that do not meet these conditions or are deemed abusive.
10. Children’s Privacy
App not targeted at under-13s; no knowing collection from children.
11. International Transfers
Data may transfer to US/Bangladesh/EU servers using Standard Contractual Clauses (EU) or adequacy decisions.

Updates will be posted on this page with a revised “Last Updated” date, and continued use of the App after changes constitutes acceptance of the updated policy.
